I've discovered what it is. It's to do with the eyebrows of the avatar. No eyebrows (for skins with eyebrows already will conflict and also using the default eyebrows will conflict. Try yourself, place custom eyebrows on and it vanishes remove the eyebrows and that strange t-shirt appears. I hope it get's fixed soon.
